A 2024 systematic review found significant improvements in pain and function in lumbar facet joint pain, but the effects on overall quality of life remained uncertain. Another recent review described the evidence as moderate rather than conclusive. Patients should treat RFA as a possible treatment option, not a guaranteed solution.

What RFA Cannot Treat?

RFA does not treat every cause of back pain. People with pain caused by nerve-root irritation may also benefit from learning whether physical therapy helps sciatica, since this condition differs from facet joint pain. What Is Radiofrequency Ablation for Back Pain?

A physician places a needle adjacent to the selected nerve and delivers radiofrequency energy via an electrode. The goal of radiofrequency ablation RFA is to reduce the nerve’s ability to send pain signals to the brain.

The heat creates a controlled lesion in a precisely targeted area. This lesion interrupts pain signaling without treating the entire spinal nerve or correcting the joint changes that caused the pain. The targeted nerve may recover or regrow over time.

Facet Joints and Medial Branch Nerves

Facet joints are small paired joints between the vertebrae that guide spinal movement. Medial branch nerves carry sensory information from these joints toward the brain. When testing identifies a facet joint as the likely source of pain, the related medial branch nerve may become the target of an RFA procedure.

What Can Radio Frequency Spine Treatment Address?

Radiofrequency spine treatment most often addresses specific forms of joint-related neck or lower back pain. It may also target nerves associated with the sacroiliac joint near the pelvis. It is not a general treatment for all spinal pain.

Radio Frequency Treatment for Lower Back Pain

Radiofrequency treatment for lower back pain may be considered for lumbar facet pain associated with arthritis or other degenerative joint changes. Symptoms may include localized aching that increases with standing, spinal extension, or rotation. Symptoms and imaging alone cannot confirm that the facet joints are causing the pain.

Cervical Ablation for Neck Pain

A person may qualify when chronic pain continues despite appropriate conservative care and testing supports a specific spinal joint as the pain source.

A pain specialist reviews the symptoms, medical history, medications, imaging, prior treatments, and responses to diagnostic blocks. A physician trained in interventional pain procedures usually performs the treatment.

Signs RFA May Be Considered

A clinician may consider ablation for back pain when several factors are present:

  • Persistent facet or sacroiliac joint pain
  • Symptoms linked to spinal movement or joint loading
  • Limited improvement with exercise, medication, or injections

How Long Does Back Ablation Last?

Pain relief often lasts six to twelve months, but results may be shorter or longer. Some people experience relief for several years, while others receive little benefit. The likely duration depends on the diagnosis, the procedure, and the individual response.

Nerve Regrowth and Repeat Treatment

The treated nerve can regrow and regain its ability to transmit pain signals. A physician may consider another procedure if similar joint pain returns and a new assessment supports the same diagnosis. Repeat treatment should not occur automatically without reviewing new or changing symptoms.

Physical therapy does not perform or replace RFA. Physical Therapy Before RFA

A PT evaluation may include a health and symptom history, movement screen, strength testing, mobility testing, and goal setting. One-on-one care delivered by a licensed physical therapist allows the plan to change based on the patient’s response.

Manual therapy or the Graston Technique may be considered when soft-tissue restrictions are relevant to the examination.

Rehabilitation After RFA

As part of a structured rehabilitation program, therapeutic exercise may begin with controlled movement and low-load strengthening. Later phases can add endurance, lifting, balance, and work- or recreation-related tasks. 

Pilates-based therapeutic exercise is one possible approach for improving core control, mobility, and gradual return to activity.

Less pain does not automatically restore strength, mobility, or physical capacity. Structured follow-up, reassessment, and home exercise carryover help identify remaining limits. The goal is to use reduced discomfort as an opportunity to rebuild function at an appropriate pace.

RFA, nerve blocks, injections, physical therapy, and surgery have different purposes:

  • Nerve blocks can test a suspected source of pain or provide temporary relief.
  • Physical therapy addresses movement, strength, function, and activity tolerance.
  • RFA interrupts pain signals from selected sensory nerves.
  • Steroid injections aim to reduce inflammation and may provide temporary relief.
  • Surgery may address structural problems that require decompression, stabilization, or repair.

Research comparing RFA with steroid injections has produced varied results, and the right choice depends on the diagnosis and treatment goals. Some patients use more than one approach at different points in care.

What Should You Ask Your Provider?

Ask which structure is believed to cause the pain, how the diagnosis was confirmed, and what outcome would count as a successful result. Discuss preparation, sedation, expected recovery, restrictions, side effects, alternatives, and the possibility of repeat treatment. You should also ask when you can safely resume exercise and physical therapy.
